Ankle and Foot Centers of Missouri, P.C. welcomes you to our office. After you set-up an appointment, there are some specifics regarding patient information and information about our procedures for new and existing patients, which you should review.

New Patients

If you are a new patient, there are a few registration forms you will need to complete. As with any new appointment with a physician, it’s important that we obtain patient demographics, patient history, authorization to release medical benefits, and protected health information consent forms. You will also want to review and complete our financial policy form.

For your convenience, please complete and bring the completed forms with you at your appointment.

When you come to your first appointment, remember to bring these items:
